EVENT POLICY INFORMATION
Covid-19 Liability Release Form RequirementFor in-person chapter events, we require attendees to read and sign a release form, stating you have no symptoms and that you have not been in contact with anyone who has tested positive for covid-19. You can view and download the form HERE. Registration FeesRegister online using your MasterCard, Visa, American Express, Discover. We do not accept checks. Walk-ins are welcome at our in person events, however meals and seating is not guaranteed. CE CertificatesTo obtain your CE certificate for an online event, you must remain logged into the event for the entirety of the presentation. For in person events, you must sign-in and sign-out at the event. All attendees must complete a program evaluation, which will be emailed within 1 week after the event. Upon completion of the evaluation, the certificate will download to your computer. You need to add your name and license number and save it. If you miss any of the presentation, you will not be eligible for CE credits. Refunds/Cancellations
GrievancesSCV-CAMFT works hard to assure fair treatment for all participants and attempts to anticipate problems. There may be occasional issues which will require intervention and/or action on the part of the board of directors. Grievances should be put in writing and mailed to the board at SCV-CAMFT Board, P.O. Box 60814, Palo Alto, CA, 94306.
